1. I understand that I am on a ninety (90) day probationary period and that any accident, incident, or violation of federal, state, and municipal laws or regulations, or violations of company policies or procedures may result in my immediate termination.

2. I understand that Patriot Underground, LLC offers health benefits, however, they are at my own expense and I am not obligated to carry them at any time. These benefits will be available immediately upon completion of the ninety (90) day trial period.

3. I understand that Patriot Underground, LLC offers vacation pay upon completion of my ninety (90) day trial period. For any holidays recognized by Patriot Underground, LLC, I will be compensated for 8 hours; I also understand that I must work the business day before the holiday and the business day after the holiday to be eligible for holiday pay. Recognized holidays: New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, Christmas Eve, Christmas Day

4. I understand that under Texas law, my employment is "at-will" and may be terminated by the employer or myself at any time.

5. I will perform my job duties as assigned and in the manner directed by my supervisor.

6. I understand that if I am not available to work within the first 30 business days of my employment, I will be terminated.

7. Upon suspicion of theft of property, located on the premises or within the Patriot Underground, LLC worksite, the company reserves the right to register your person or your property for such property. Only that property will be the object of the search.

8. I understand that all requests for time off (paid or unpaid) must be submitted to the supervisor two weeks prior for approval.

9. I understand that if for any reason I miss two or more consecutive days (not pre-approved) and do not call my supervisor, it will be considered my resignation.
